3-STEP PROCESS:

Step 1: Referral or Inquiry

Your physician, hospital discharge planner, or family member contacts us. We accept physician referrals and self-referrals.

Step 2: Assessment

A Care Luxe nurse visits the patient at home to complete an initial assessment and confirm eligibility and care needs.

Step 3: Care Begins

We build a personalized plan of care with the physician and begin skilled services — bringing clinical excellence right to your door.

Care Luxe Home Health is a Medicaid-certified agency. Eligible patients may receive covered services at no out-of-pocket cost. Contact us to verify eligibility.
